To save you time at the race, the USA Cycling Rider Release Forms and the City of Pittsburgh Insurance Release Form for the ACA Summer Criterium Series are available online. Please follow the instructions below.

Instructions for USA Cycling Form

• Print one copy of the Tuesday Night USA Cycling Standard Release form OR one copy of the Wednesday Night USA Cycling Standard Release form on your printer
• Sign and date the USA Cycling form with the date of the race -- you MUST submit a USA Cycling Form for every race!
• Bring a signed and dated form to every race you enter

Instructions for City Of Pgh Form

• Print one copy of the City of Pittsburgh Insurance Waiver
• Sign and date the City Form
• Bring the signed form to your first race

Please note that these USA Cycling forms are applicable only for the ACA Summer Criterium Series, because they have the specific dates inserted. They should not be used for other USA Cycling races. Click here for the standard, blank USCF forms.
